Social Services
The goal of the Social Services is to assist patients and their families with social, emotional, and environmental problems associated with hospitalization. Functions include:
- General Information> - Referral to community and financial resources, including a Medicaid Enrollment Program, explanations of hospital routine and procedures to patients, legal services, and educational programs.
- Environmental Problems - Assistance with transportation, employment, child care, sitter services, housing, medical equipment and supplies.
- Financial Assistance - Assistance with medical expanses, insurance programs, income replacement.
- Counseling and Guidance - Assistance in solving illness related problems.
- Transfer Planning - Transfers to other hospitals, rehabilitation facilities, long-term care centers, etc.

- Discharge Planning - Referral to nursing home, home health services, home care and nutrition services, outpatient therapy and vocational rehabilitation services, and special supplies and equipment.
- Follow-Up-Services - consultation to assure quality post-hospital care.
You may request to visit with a Social Services Director while in the hospital by asking your nurse or physician to notify the department, or as an outpatient by reporting to the Admissions Department and requesting that Social Services by paged or by phoning the hospital switchboard.
